EMCF Provider Flu Vaccine Status

Each year we are required to report to West Tennessee Healthcare information for our providers regarding the flu vaccination:

If you received the flu vaccination, at what facility was it given? (WTH facility or other?)
If you received it at another facility, documentation must be provided.
If you choose to decline the flu vaccination:
a) you must sign a flu declination form
b) you are required to wear a mask beginning December 1, 2024 - March 31, 2025
